SCHEDULING
Your general dentist may feel that you need to be evaluated and/or treated by an endodontic specialist, due to your symptoms, radiographic (X-Ray) findings or clinical exam. He or she may call our office to schedule an appointment or may refer you to our office and ask you to contact us. Our Scheduling Coordinators will gather information necessary to insure you are scheduled as quickly and efficiently as possible. They are also available to answer any questions you may have in regard to your visit or our services. If you are experiencing pain or are in an emergency situation, every effort will be made to see you as soon as possible.
In an effort to schedule efficiently, we will ask for information such as the tooth number to be evaluated, history of dental treatment in the affected area, symptoms, etc. and may ask to be provided a radiograph (X-Ray) taken by your general dentist. This information should allow us to schedule you most appropriately, minimizing the number and duration of your visits in our office.
Please click the button below to request an appointment or call our office at (850) 877-0536 to schedule or ask any questions.
FIRST VISIT
On the day of your appointment, you will be asked to fill out our patient registration forms, including your health history. Your visit will consist of a consultation explaining your diagnosis and treatment options. If treatment has been scheduled at time of consultation, we will provide treatment that day. If you are scheduled for a consultation only, we will require a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
Your referral slip and any X-rays, if applicable.
A list of medications you are presently taking, the dosage, and what you are taking them for.
If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or legal guardian must accompany patients under 18 years of age, to their initial visit.
Please alert our office prior to your appointment if you have a heart condition or any artificial joints, as these may require you to pre-medicate with antibiotics. We also need to be alerted if you have a medical condition that may be of concern prior to surgery, such as, diabetes or high blood pressure, or if you are taking any of the following: heart medications, aspirin, or anticoagulant therapy.
